Allied Health South Lincolnshire Ltd (AHSL) is looking for 2 x 0.6WTE( pro rata) coordinator posts to support our specialist neighbourhood nursing team.

We are seeking a dedicated and compassionate Care Coordinator to join our team. The Care Coordinator will play a vital role in ensuring that patients receive the highest quality of care by facilitating communication between patients, families, and healthcare providers. This position requires strong organisational skills and the ability to manage multiple tasks efficiently in an office environment.
The post holder will play a key role in supporting and working with the SNN team within SLR PCN to identify and support people to reduce the risk of unplanned hospital admissions and effectively support those individuals in the community. They will provide additional capacity to the SNN team, to allow them to focus on providing proactive care to patients living with frailty and support the SNNs in working with the frail/elderly and those with long-term conditions.
They will work closely with the SNN team, GPs, and practice teams, to provide an all-encompassing approach to personalised care and enable people to navigate through the health and care system, making sure that appropriate support is made available to people; supporting them to understand and manage their condition and ensuring their changing needs are addressed. They will support people to access the services and support they require to meet their health and wellbeing needs, helping to improve people's quality of life.
The post holder will work with a diverse range of people from different cultural and social backgrounds & will be able to work confidently and effectively in a varied, and sometimes challenging environments. They will have excellent interpersonal and communication skills, and be organised, patient and empathetic and have experience of working in health, social care or a supporting role which involved direct contact with individuals, families and/or carers.
Responsibilities
The Care Co-ordinator with be a pro-active member of the South Lincs Rural Primary Care Network(SLRPCN) Neighbourhood Team
To Support the SNN to proactively identify vulnerable and at-risk patient groups.
Be an active part of the Integrated Neighbourhood Team, liaising between providers of care to ensure people receive the right care, at the right time, first time by the right people to promote seamless transition.
Support the SNN's with the monthly Neighbourhood Multidisciplinary team meetings (MDT) by; recording minutes, follow up actions identified within the individual's clinical records.
Have an awareness of local services, & continually build on knowledge of services, to enable supported signposting of people with identified need, and sharing of information gained with the SNNs & practice staff.
To receive and action non-clinical tasks from the SNN team to assist them with referrals to other organisations and professionals/wider MDT.
Assist the SNNs where they have identified people at risk of loss of independence or admission to hospital to put plans in place to support them to remain in their usual place of residence.
As directed by the SNN, follow up on communications received from other professional organisations, regarding the changes in condition of people to support the SNN to respond proactively to potentially unmet needs.
As directed by the SNN, contact individuals post discharge from hospital to ensure needs are being met.
Complete joint visits with SNNs on request to ensure the safety of all involved.
As directed by the SNN, complete follow up contact with individuals & their family/carers to ensure needs have been met.
Maintain accurate and up to date records of patient contacts using GP record systems and other IM&T systems relevant to the role i.e. entering notes onto Systmone / EMIS/VMDT using agreed read codes.
Monitor tasks via clinical systems and the South Lincs rural (SLR) cover email inbox in the absence of SNN and escalate to covering SNN as appropriate. Cover other email boxes on request as needed.

Requirements
Previous experience in a healthcare or office setting is preferred.
Strong organisational skills with the ability to manage multiple priorities effectively.
Excellent verbal and written communication skills, with a focus on empathy and understanding.
Proficient in using office software applications and electronic health record systems.
Ability to work collaboratively within a team while also being self-motivated.
A commitment to maintaining patient confidentiality and adhering to ethical standards in healthcare.
