Rockingham Forest PCN is a large, forward-thinking Primary Care Network compromising 3 practices across Corby, Kettering and Oundle covering a population of c.67k patients

Job Introduction
As the Personalised Care Team Lead, you will oversee and support the Social Prescribing Link Workers within Rockingham Forest PCN. This role is about empowering patients to take control of their health and wellbeing. Our Link Workers help people with a wide range of needs--whether they are managing long-term conditions, experiencing low mood or loneliness, or dealing with complex social challenges.
You will be responsible for the day-to-day supervision of the team, providing guidance through the development and review of processes, setting clear criteria, and strengthening connections with local community and voluntary groups.
Alongside your leadership responsibilities, you will also work directly within a personalised care role--either as a Social Prescribing Link Worker, Health and Wellbeing Coach, or Care Coordinator. Working as part of our multidisciplinary team, you will support individuals to access non-medical services, build their confidence, and connect with community resources that address the wider factors affecting their health and wellbeing.
What you will do
As the Personalised Care Team Lead, you will play a key role in shaping and supporting personalised care across the PCN. You'll lead a dedicated team of Social Prescribing Link Workers, Health & Wellbeing Coaches, and Care Coordinators, providing day-to-day supervision, guidance, and opportunities for staff development.
Working closely with GP practices, community partners, and voluntary organisations, you'll help patients access the right support at the right time -- whether that's connecting with local services, building confidence to manage their health, or addressing wider social challenges.
Alongside your leadership role, you'll also work directly with patients in a personalised care capacity, ensuring you remain connected to the people and communities you serve. By collaborating with the Digital & Transformation Manager, you'll drive new initiatives, refine existing processes, and expand the support available, helping patients to live healthier, more independent lives.
Essential Criteria
NVQ Level 4, Advanced Level or Equivalent qualification (in health and social care, or another relative subject) or working towards.
Experience of working with the public, preferably in the social care or healthcare sectors.
Experience of working within a personalized care role within the NHS.
Experience of providing supervision; ability to lead and engage with teams.
Excellent people skills and the ability to influence change.
Good ability to use IT functions and a familiarity with GP systems and its reporting functionality.
Good understanding and awareness of the requirements of supporting vulnerable communities to access suitable services.
Access to a vehicle to travel across the PCN as required.
