Job Title: Band 5 ARRS equivalent Pharmacy Technician - Primary Care Network (PCN)

Salary: Band 5 ARRS EQUIVALENT (31,049 - 37,796 per annum, pro rata)
We are excited to offer an opportunity for a motivated and enthusiastic Pharmacy Technician to join our dynamic and growing team within the Hertford & Rurals Primary Care Network (PCN). Key Responsibilities:
Medicines Optimisation: Support the implementation of safe and effective systems for medicines optimisation, medicines reconciliation post-discharge, effective medicines administration, repeat prescribing, and reducing medicine waste.
Patient Interaction: Respond to medicines related patient queries both face-to-face and over the phone, referring complex issues to the pharmacist or GP.
Clinical Correspondence: Review clinical correspondence, such as discharge letters, and Reconcile medication to ensure safe and accurate transfer of care in a timely manner.
Medication Audits: Participate in medication audits, focusing on high-risk drugs, shared care agreement medicines to ensure effective monitoring. Liaising with pharmacists and GPs for reviews. Share outcomes of clinical audits to facilitate improvement,
Advice and Information: Liaise with the pharmacy team to provide advice to GPs and other multidisciplinary team members, and communicate with patients to improve prescribing safety, quality, and cost-effectiveness.
Prescribing Budgets: Have an awareness of the prescribing budget and identify areas for cost improvement where appropriate. Assist practices in monitoring prescribing targets as per the PCN DES., Patient Reviews: Conduct patient medication reviews as identified by PCN pharmacy team, including physical checks like weight, BMI, and blood pressure, and establish patient medication compliance.
Skills and Qualifications:
Registration: Must be a registered pharmacy technician with the General Pharmaceutical Council (GPhC).
Experience: Previous experience in a pharmacy setting, preferably within primary care.
Communication: Effective communication skills to interact effectively with patients and healthcare professionals.
Attention to Detail: High level of accuracy in reviewing and managing medication records.
Teamwork: Ability to work collaboratively within a multidisciplinary team.
Location: Based across multiple practices within the PCN.
Hours: Full-time (37.5 hours per week) or part-time, depending on the needs of the PCN.
Supervision: Work under the supervision of a pharmacist and report to the PCN Manager and Lead PCN Clinical Pharmacist.
