LOCC PCN is a forward-thinking Primary Care Network made up of 4 GP Practices, working together to improve the health and wellbeing of our local communities. We are committed to delivering holistic, patient-centred care and supporting the wider determinants of health.

About the Role



We are looking for a compassionate, motivated, and proactive Social Prescribing Link Worker to join our multidisciplinary team. The role involves working closely with GPs, healthcare professionals, and voluntary/community organisations to support individuals in accessing non-medical services that improve health and wellbeing.

You will play a key role in empowering patients to take control of their health by connecting them with appropriate local services, groups, and activities that address social, emotional, or practical needs.

Key Responsibilities



Work with referred patients to identify their needs and co-produce personalised care plans. Support individuals to access community-based activities and services (e.g., debt advice, housing support, physical activity groups, social groups). Build strong relationships with voluntary and community sector organisations. Maintain accurate records using clinical systems (e.g., EMIS) and contribute to outcomes reporting. Work as part of a multi-disciplinary team across the PCN.

About You



We are looking for someone who is:

Passionate about improving health and wellbeing through non-clinical approaches. An excellent communicator with strong interpersonal skills. Able to work independently and manage a caseload effectively. Confident in liaising with a wide range of health, social care, and community stakeholders.

Essential Criteria:



Experience working with people in a support, advice, advocacy, or social care role. Knowledge of local health, social care, and voluntary services. Ability to maintain professional boundaries and handle sensitive information with confidentiality. Strong IT skills and familiarity with NHS systems or willingness to learn.

Desirable:



Previous experience as a Link Worker or in social prescribing. Understanding of the wider determinants of health and health inequalities.
