We are seeking a dedicated and compassionate healthcare professional to join our team in Torquay This role offers an opportunity to provide high-quality patient care in the home for a client with dementia. The successful candidate will be responsible for supporting patient wellbeing through clinical expertise and empathetic communication, ensuring compliance with healthcare standards and protocols.
Duties
Deliver comprehensive patient care, including assisting with daily activities including bathing, toileting and medication administration.
provide companionship, incouriment and support
Transport patient to group meetings via our WAV
Maintain detailed and accurate patient records in accordance with organisational standards.
Collaborate with multidisciplinary teams to develop personalised care plans.
Educate patients and their families about health conditions, treatments,
Monitor patient progress and report any changes or concerns to supervising medical staff.
Qualifications
Proven experience of care for patients with dementia in the home setting
Knowledge of anatomy, medication administration, and patient care procedures.
Compassionate approach with excellent communication skills.
Relevant certifications or licences in care support, also full driving licence.
Ability to work independently and as part of a multidisciplinary team. This position is ideal for healthcare professionals committed to delivering exceptional patient-centred care within a supportive environment.
