We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The Pharmacist will work closely with healthcare professionals in various settings, including Intensive Care Units (ICU), to optimise patient outcomes.
Responsibilities
Dispense prescribed medications accurately and efficiently, ensuring compliance with legal and professional standards.
Provide medication administration guidance to healthcare staff and patients, ensuring understanding of dosage and potential side effects.
Conduct thorough reviews of patient medication histories to identify potential interactions or contraindications.
Collaborate with physicians and other healthcare professionals to develop comprehensive care plans tailored to individual patient needs.
Monitor patient progress and response to medications, making necessary adjustments in consultation with the healthcare team.
Educate patients on proper medication usage, potential side effects, and lifestyle modifications to enhance treatment efficacy.
Maintain accurate records of prescriptions and patient interactions in accordance with regulatory requirements.
Stay updated on advancements in pharmacology, medication therapies, and best practices in patient care.
Skills
Strong knowledge of medication administration techniques and protocols.
In-depth understanding of human anatomy and physiology as it relates to pharmacotherapy.
Excellent patient care skills, demonstrating empathy and effective communication with diverse populations.
Experience working in an ICU setting is highly desirable, showcasing the ability to manage complex medication regimens under pressure.
Strong analytical skills for evaluating patient data and making informed decisions regarding medication management.
Ability to work collaboratively within a multidisciplinary team while maintaining a high level of professionalism.
