Overview

We are seeking a dedicated and knowledgeable IP Pharmacist specialising in Diabetes to join our healthcare team. The ideal candidate will play a crucial role in ensuring the safe and effective use of medications for our patients. This position requires a strong understanding of medication administration and patient care. The Pharmacist will work collaboratively with healthcare professionals to optimise patient outcomes and provide exceptional pharmaceutical care.

Responsibilities

Provide expert advice on medication management to patients and healthcare professionals. Monitor patient progress and adjust medication regimens as necessary based on clinical guidelines. Educate patients about their medications, including proper administration techniques and potential side effects. Collaborate with multidisciplinary teams to develop comprehensive care plans for patients. Maintain accurate records of prescriptions and patient interactions in accordance with organisational policies. Stay updated on the latest developments in pharmacology, therapeutics, and relevant clinical practices.

Qualifications

A valid pharmacy degree from an accredited institution. Registration with the General Pharmaceutical Council (GPhC) or equivalent body. Strong knowledge of medication administration practices. Excellent understanding of human anatomy and physiology as it relates to pharmacotherapy. Proven experience in providing patient care and counselling on medication use. Strong communication skills, both verbal and written, with the ability to convey complex information clearly. Ability to work effectively as part of a team while also demonstrating initiative in problem-solving.
