Job Summary

We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in ensuring the safe and effective use of medications, providing exceptional patient care, and contributing to the overall health of our community. The Pharmacist will collaborate with healthcare professionals and patients to optimise medication therapy and improve health outcomes

Key Responsibilities:

Dispense prescription medications accurately and efficiently. Provide medication counselling to patients regarding proper use, potential side effects, and drug interactions. Perform pharmacy services including the hypertension case-finding service and the Pharmacy First service (urgent supply, minor ailments, and clinical pathways). Monitor patient responses to medication therapy and make appropriate adjustments as needed. Maintain accurate records and documentation. Ensure compliance with all relevant laws, regulations, and standard operating procedures. Participate in quality improvement initiatives to enhance patient outcomes.

Qualifications:

Registration with the General Pharmaceutical Council (GPhC). Accredited and willing to perform flu and COVID-19 vaccinations. Completed training for Discharge Medicines Service (DMS) and the Pharmacy First Service. Excellent attention to detail and problem-solving skills. Proficient in clinical knowledge. Strong communication and interpersonal skills.
