Patient Care & Service Delivery: Act as the first point of contact for patients, carers, and healthcare professionals regarding medicines-related queries. Support the coordination and scheduling of medication reviews, polypharmacy reviews, and structured medication reviews led by pharmacists. Assist with managing medication-related recalls and monitoring for long-term conditions (e.g., blood tests, blood pressure monitoring, medication safety checks). Proactively follow up with patients to ensure timely completion of medication-related actions.
Liaise with community pharmacies, hospitals, care homes, and other healthcare providers to ensure safe and seamless care transitions. Support delivery of public health campaigns and initiatives relating to medication safety, adherence, and healthy lifestyles. Administrative Support Provide administrative support to the clinical pharmacy team to enable effective delivery of pharmacy-led services. Ensure accurate documentation and timely data entry into clinical systems (EMIS/SystmOne).
Process incoming and outgoing communications related to medication queries, prescription requests, and patient follow-up. Assist in the collection, reporting, and analysis of data to support medication safety audits, service development, and Quality and Outcomes Framework (QOF) indicators. Maintain electronic patient records in accordance with data protection legislation, GDPR, and practice policies. Team Working & Communication: Work collaboratively with the wider general practice team including GPs, nurses, and care coordinators to ensure high-quality patient care.
Attend practice and PCN meetings as required to support integrated working. Participate in training and development opportunities to support professional growth and service delivery.
