Job summary



You will be employed as a Pharmacy Technician within the Primary Care Network (PCN) at one of the four practices which are based across Pontefract and Knottingley supporting the pharmacists in ensuring patients receive safe, accurate, and timely medications. Pharmacy technicians assist with tasks such as preparing and dispensing prescriptions, managing inventory, and providing advice and support to patients within their sphere of competency and under the supervision of a clinical pharmacist and/or General Practitioner within the GP Practice. Role holders will write up notes and record information using system protocols and may draft processes, guidelines, analyse data and produce reports for their own or the use of others. Role holders have responsibility for planning their workload and need to be able to respond to changes in demand and priority. Their role may involve more complex scheduling and coordinating tasks across multiple stakeholders or projects. They are less closely supervised than at Level B. They may attend networks and partnerships outside the immediate team to share good practice and resources. They are trained and competent in technical aspects of the role which may include clinical procedures and require ongoing CPD.

Main duties of the job



The role holder will be required to demonstrate that they have a range of general experience of working in a health and social care setting (preferably with some primary Care knowledge) along with experience in at least one other setting, for example a variety of primary care practices, care homes, long term care, palliative or end of life care or community, hospital or industry pharmacy.

Job description



Job responsibilities



To help deliver and support the PCN and practices in delivering targets for quality improvement projects as well as helping them achieve impact and investment targets. Help provide advise and support on medicines management and training to support practice staff, GPs, Clinical Co-ordinators, and other team members. To deliver audits, medication changes, medicines management reviews and activities in the practices and care homes as part of the PCN. To assist in analysing and monitoring PCN prescribing data and offer support on prescribing indicators. To help ensure optimal medication use through shared care management and maximise medication optimisation and effective medicines administration and support medicine reconciliation and reviews for patients and care homes. To effectively council patients to safeguard the use of their medicines effectively and aid synchronisation of medicines with primary and secondary care, ensuring community pharmacies are informed about changes and safe medicine delivery to safeguard patients. As well as supporting clinical pharmacists to carry out efficient structured medication reviews by arranging the correct monitoring is in place prior to the reviews. Identify blood tests, recalls and reviews using established protocols and forward these to the appropriate clinician. To provide patients with knowledge of social care and public health needs including sign posting, and lifestyle advice to reduce health inequalities. With referral to PCN Health and Wellbeing Coaches where relevant. To help support shared care and local protocols to reduce inappropriate prescribing, as well as support local and national safety agendas. Support the clinical team in delivering QIPP, QOF and local commissioned services. Ensure the medicines optimisation processes are followed effectively. To help reduce medication wastage through effective ordering and setting up safe electronic repeat dispensing. Support practice prescribing budgets by on-going monitoring, audit and analysis of drug use and support practices in reviewing, developing, and enhancing cost-effective prescribing. This includes staying aware of latest QIPP and other cost targets To help support integration across community pharmacy, practice, care homes and other primary care settings. As well as supporting the practice reception teams in managing prescription queries to help identify more clinical complex requests are directed to GP's and clinical pharmacists.
Other duties may be required to meet the responsibilities of the post and the requirements of the organisation and the primary care networks.

Person Specification



Experience Essential



The role holder will be required to work effectively as part of a team and be able to demonstrate the ability to problem solve and respond to sudden unexpected demands or situations. IT and report writing skills are also an essential element of the role, together with the ability to manipulate information and pull together date, letters and reports with attention to detail and the ability to work accurately, identify errors quickly and easily.

Qualifications



Essential



BTEC or NVQ3 in Pharmaceutical Sciences or equivalent with induction experience and relevant work experience AND current registration with the General Pharmaceutical Council. Willing and committed to undertaking the CPPE Primary Care Pharmacy Education Pathway

Desirable



Has already completed their CPPE Has at least 2 years experience of working in a primary care setting as a pharmacy technician
