This role will be extremely rewarding, and requires a motivated, dynamic individual who is experienced in working with a range of stakeholders and has a track record for service development and delivering to agreed targets.



You will be a GPHC registered pharmacy technician with excellent interpersonal and IT skills. This is a permanent full-time post although part time hours or a job share will be considered.



The working hours are 37.5 hours per week Monday to Friday with limited late shift duties and Bank Holiday commitments. This provides a fantastic work life balance whilst being a valued member of the Pharmacy Medicines Optimisation Team. The post holder may be required to work flexibly to meet the needs of the service.



JOB PURPOSE

To work as part of the pharmacy team at MPFT to ensure the safe, effective and economical use of medicines. To work with clinical pharmacists, the dispensary manager and other medicines management pharmacy technicians and pharmacy support workers in providing and developing a comprehensive medicines optimisation service to the wards and clinics. Lead the provision and coordination of the Trust Pharmacy led Medicines Reconciliation service in designated areas to ensure the successful implementation of Trust Policy for Medicines Reconciliation. The aim of the Pharmacy Team will be to facilitate the Trust achieving the NICE/ NPSA target of 90% newly admitted patients having their medicines reconciled within 24 hours. To provides counselling to patients/and carers as needed. To participate in discharge planning to ensure the timely dispensing of discharge medication for patients on the medicines management wards undertaking the final check of patients' own drugs before discharge. Work to promote safe, legal, rational and economic use of medicines by all users of the pharmacy service. To support the implementation and delivery of current and future digital based innovations in pharmacy practice. To assess patients' own drugs for use on the wards and document the supplies

available on patients' drug charts.

Identifying charts that have not yet been clinically screened by a pharmacist.

KEY RESPONSIBILITIES



Main duties and responsibilities



Prescription processing



1. The post holder will dispense and/or supervise the dispensing of medication to inpatients/outpatients, adhering to the current departmental policies and standard operating procedures regarding the legality, interpretation of the prescription and the accuracy of the dispensing. Other factors in the process are the timeliness of the dispensing, the provision of information and the referral of any queries related to the above to a pharmacist. Provision of information includes counselling patients, either at the pharmacy hatch or on the ward, to ensure they understand when and how to take their medicines and any additional information related to their prescription they may require.



2. The post holder will be expected to take on the role of accuracy checking technician (ACT), i.e. checking for accuracy prescriptions dispensed by other members of the dispensing staff which have previously been checked for clinical appropriateness by a pharmacist.



3. The post holder will organise with doctors (or their secretaries) the supply of repeat prescriptions (clozapine) for those patients whose prescriptions have expired.



4. The post holder will be involved in the dispensing and process support for Homecare products



5. The post holder will be involved in the dispensing and process support for Clinical trials



6. Recording and reflection of any errors made as part of CPD.
