Job Description

Medicines Management and Optimisation Support the Practice Pharmacist in ensuring safe, efficient, and effective systems for repeat prescribing. Participate in medication audits and quality improvement initiatives. Assist in implementing prescribing changes and medicines optimisation plans. Monitor prescribing trends and support initiatives to reduce medicines waste and improve safety.

Repeat Prescribing and Medicines Reconciliation Support administrative staff with the preparation and processing of repeat prescriptions to ensure accuracy and timeliness. Review and reauthorise repeat prescriptions under appropriate guidance. Conduct medicines reconciliation following hospital discharge or care transfers, updating medical records as required. Identify discrepancies and resolve them in collaboration with the Practice Pharmacist and GPs.

Patient-Facing Support Provide information and advice to patients regarding their medicines, ensuring understanding and adherence. Respond to medication queries from patients and staff in a professional and timely manner. Practice Support and Collaboration Provide supervision, training, and guidance to administrative staff involved in repeat prescribing. Liaise with community pharmacies, hospitals, and other healthcare providers to support seamless patient care.
