An exciting opportunity has arisen for a Pharmacy Technician to join our developing primary care network team within South Bucks PCN at Iver Medical Centre , we provide high quality support to our patients and are looking for an enthusiastic individual to join the team to support us in achieving this.
The network is already experienced in working with clinical pharmacists and technicians this role will provide a great opportunity to join a committed team as well as having access to national training through the NHS England Clinical Training Pathway and be part of a local network of clinical pharmacists/technicians.
We are looking for a collaborative, motivated and forward-thinking technician. The role will involve supporting medicines management by working within the general practice setting, with the support of Senior Clinical Pharmacist.
You will provide specialist medicines management advice to patients and other healthcare professionals, to promote the safe, effective and high-quality, cost-effective use of medicines in all prescribing decisions. The role will also focus on improving the repeat prescribing process, relieving clinical workload pressures.

Key Responsibilities
To undertake medicines reconciliation in accordance with practice PCN protocols and to update patients' medical records / prescribing accordingly.
To support the achievement of the practice's PCN prescribing Quality and Outcomes Framework targets
Working with the Clinical Pharmacy team to contribute to the review and development of repeat prescribing protocols / Standard Operating Procedures to improve the efficiency and effectiveness of repeat prescribing systems.
To provide advice to GPs, staff and patients in changes to prescribing to support the improvement of prescribing safety, quality and cost effectiveness.
To participate in medication audits.
To assist in the appropriate monitoring and management of prescribing the budgets and to liaise with the medicines management team at the CCG.
To prepare evidence based resources and information to support the implementation of rational cost-effective prescribing.
To support the PCN Based Clinical Pharmacist to provide medication reviews, patient information leaflets, medicine awareness; and to undertake community visits as appropriate.
To take a proactive approach to the safe handling of prescriptions, including assessment of risk and making recommendations for improvement
To respond to medication queries from patients and staff in a professional manner referring to the appropriate Clinician in accordance with practice PCN protocols.
