Job Overview


To provide high-quality support for the delivery of medicines optimisation and management for the LCW run GP Practice and across the wider LCW services, ensuring patient safety, compliance with medicines regulations, and the efficient use of resources. The postholder will assist in implementing prescribing safety initiatives, managing medicines supply and supporting audits.

Job descriptionJob responsibilities
1. Delivery at GP Practice

Assist in conducting medication reviews, focusing on patients with polypharmacy and long-term conditions. Support the implementation of Optimised Assurance Framework (OAF) standards, including responding to drug alerts and managing safety protocols. Prepare materials and assist with educational sessions for healthcare professionals, including arranging resources for external speakers. Perform high-risk drug monitoring reviews, ensuring compliance with safety protocols and addressing any prescribing concerns under the guidance of the Lead Clinical Pharmacist. Contribute to the provision of safe and effective pharmacy services. Undertake medicines reconciliation in line with practice protocols to improve efficiency and the effectiveness of prescriptions. Respond to patients queries face-to-face and via phone, referring complex problems to the pharmacist. Participate in patient medication reviews Check the accuracy of medicines on prescriptions to ensure compliance and safety. Participate in medication audits, identifying areas for improvement and supporting implementation of changes. Assist in monitoring prescribing budgets and preparing evidence-based resources to support cost-effectiveness.
2. LCW-Wide Medicines Management Support

Assist with the management of Controlled Drugs, ensuring secure storage, accurate records, and compliance with Home Office and CQC regulations under the supervision of the CD License holder. Conduct medicines-related audits and stock checks across LCW services, ensuring adherence to policies and regulatory standards. Collaborate on the development and review of medicines-related policies. Provide technical support for the investigation of significant incidents or near misses related to medicines, under the guidance of the governance team. Support the procurement, storage, and distribution of medicines, ensuring cost-effective and compliant processes.
3. Administrative and Operational Support

Maintain accurate and up-to-date documentation for medicines management processes, including data collection for audits and reporting. Act as a point of contact for medicines-related queries, referring complex cases to the Lead Clinical Pharmacist or governance teams as appropriate. Assist with the preparation of reports and presentations for governance meetings and training sessions. Support with the notes of the medicines management group.
4. Education and Training

Support the delivery of training programmes for clinical and support staff, ensuring awareness of medicines management best practices and regulatory compliance. Participate in personal and professional development activities, ensuring up-to-date knowledge of medicines management standards.

Job Type: Fixed term contract
Contract length: 18 months

Pay: 30,000.00-35,000.00 per year
