Job Purpose



As a Positive Behaviour Support (PBS) Practitioner, you will deliver and embed evidence-based, person-centred interventions that improve quality of life and reduce behaviours that challenge. Working within The Bridge Care's Systemic PBS Model and Framework, you will ensure support is proactive, ethical, and empowering.

As a PBS Practitioner, you will work alongside our dedicated Specialist Support Team a collaborative group of professionals committed to delivering transformative support through our tiered PBS model. Together, we drive innovation, embed best practices, and ensure every person we support has the opportunity to live safely, happily, and successfully.

Key Responsibilities



1. PBS Assessment & Planning



Conduct Functional Behaviour Assessments (FBA) and bespoke assessments. Develop and implement Positive Behavioural Support (PBS) and Behaviour Intervention Plans (BIP). Collaborate with individuals, families, and teams to co-produce personalised, person-centred strategies. Integrate Active Support and Person-Centred Planning (PCP).

2. Tiered Support Delivery



Deliver universal Tier 1 supports: communication planning, trauma-informed care, PERMA-based well-being. Provide Tier 2 targeted interventions using data-driven early detection and proactive support. Contribute to Tier 3 intensive support: crisis management, environmental modifications, ethical medication practices.

3. Embedding Systemic PBS Practices



Lead and model PBS principles and practice across all homes. Deliver coaching, mentoring, and training sessions including physical intervention and PERMA workshops. Promote consistent, high-quality support environments.

4. Monitoring, Data & Quality Assurance



Collect and analyse behavioural data (ABC charts, incident reports, quality of life metrics). Track intervention efficacy and engagement. Contribute to continuous improvement through data-led refinements.

5. Referral & Triage



Manage and triage PBS referrals. Liaise with Principal PBS Practitioner to ensure appropriate tiered support allocation.

6. Ethical Practice & Restrictive Practice Reduction



Champion ethical support through RRN and STOMP initiatives. Support reduction of PRN medication and restrictive interventions. Embed trauma-informed, dignity-focused approaches.

7. Collaboration and Leadership



Work closely with service managers, clinicians, and broader support teams. Participate in governance reviews, PBS service reviews, and workshops. Lead PERMA action planning and review sessions within services.

Key Competencies



Strong knowledge of PBS, behaviour theory, and the Biopsychosocial Model Proficient in conducting FBAs and analysing behavioural data Experience supporting individuals with learning disabilities, autism, or mental health needs Skilled in coaching and mentoring staff teams Confident leading PBS practice across multiple support tiers

Desirable Qualifications & Experience



Degree in Psychology, Social Work, Behavioural Sciences, or related field BILD-accredited or equivalent PBS certification Experience within a multi-tiered support system Familiarity with RRN, STOMP, and ethical medication practices
