Job Description

Deliver and evaluate a wide range of health interventions to meet patients health and social care needs. Promote health and well-being through individualised, person-centred care plans, especially for patients with or at risk of long-term conditions. Manage and treat long-term conditions (e.g., asthma, COPD, diabetes, hypertension) in line with national and local policies, including running dedicated clinics and reviews. Conduct NHS health checks for new patient registrations and provide general and specific health screenings and assessments.



Review medication for therapeutic effectiveness using evidence-based practice and national protocols, supporting patient adherence to treatments. Triage and prioritise health problems, intervening appropriately in complex, urgent, or emergency situations, including initiating emergency care. Support and manage womens health needs, including family planning and cervical cytology consultations. Implement and participate in adult and child vaccination and immunisation programmes, including travel vaccinations.



Provide wound care and perform appropriate tests such as blood samples, swabs, smears, ECGs and 24-hour blood pressure monitoring. Advise and educate patients on general health care, maintaining a collaborative, supportive and holistic approach at all times. Participate in patient education sessions, local health campaigns and collaborate with patient participation groups to improve health outcomes.
