We are seeking a dedicated and skilled Registered Nurse to join our Wigan North PCN team. We are a PCN of 4 practices in the Wigan North area (Aspull Surgery, Beech Hill Medical Practice, Shevington Surgery and Standish Medical Practice) and work closely together to deliver high-quality patient care.
The successful candidate will play a vital role in our PCN team, working collaboratively with multidisciplinary teams to ensure the best outcomes for patients. This position requires an individual who is passionate about clinical excellence and committed to continuous professional development.
Experience of working in general practice or care homes is essential.
The role will be employed under the Additional roles Reimbursement Scheme (ARRS) meeting the requirements outlines in the Network Contract Agreement DES.
Main Duties
The following are the core responsibilities of the PCN Nurse.
There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
The job will involve being part of a care home team providing support to the care homes allocated to the 4 practices within the PCN.
Provide a regular ward round to patients at care homes within the PCN
Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the PCN's social prescribing service and, where appropriate, other community or voluntary services
Ensuring the care homes are following the correct procedures in raising daily queries to the appropriate teams to reduce the workload on the practices.
Communicate at all levels across organisations ensuring that an effective, person-centred service is delivered
Completing and reviewing care plans for all patients within the care homes
Working alongside the other members of the care home team to ensure all medication related queries are dealt with in a safe and timely manner.
Regular communication and updates to the practices within the PCN
Attend meetings with the PCN when required, to provide updates and discuss the service
Ensure continued compliance with indemnity cover.
Practice in compliance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions and omissions at this level of practice
Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice, including when working with complexity, risk, uncertainty and incomplete information
Act on professional judgement about when to seek help, demonstrating critical reflection on own practice, self-awareness, emotional intelligence, and openness to change
Demonstrate effective communication skills, supporting people in making decisions, planning care or seeking to make a positive change
Use expertise and decision-making skills to inform clinical reasoning
Exercise professional judgement to manage risk
Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patient's health
Be a professional role model for the PCN
Maintain accurate clinical records in conjunction with extant legislation
and to ensure SNOMED CT codes are used effectively
Chaperone patients where necessary
Support the team in dealing with clinical emergencies
Support the team with all safeguarding matters in accordance with local and national policies
Secondary Responsibilities:
In addition to the primary responsibilities, the Nurse may be requested to:
Undertake overall management of a clinical team, providing guidance when necessary, acting as a mentor to students and newly qualified staff
Maintain an effective clinical staff rota ensuring all clinics are staffed appropriately
Participate in local initiatives to enhance service delivery and patient care
Support and participate in shared learning within the PCN
Continually review clinical practices, responding to national policies and initiatives where appropriate
Participate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA)
Commensurate with their role, support the development of clinical services within the PCN, liaising with external agencies as required
