This is an excellent opportunity for a nurse with primary care experience to manage a diverse caseload, run specialist clinics, and contribute to health promotion and chronic disease management in a supportive community setting.

Key Responsibilities

Provide comprehensive nursing care, including assessment, screening, treatment, and health education for patients of all ages. Manage chronic disease clinics (e.g., diabetes, asthma, COPD, hypertension, coronary heart disease). Perform treatment room duties such as wound care, venepuncture, ear irrigation, ECG recording, and blood pressure monitoring. Deliver immunisations (childhood, travel, and flu/covid) and cervical cytology screening. Support family planning, women's and men's health, and contraception services. Assist with health promotion initiatives, including smoking cessation, weight management, and lifestyle advice. Maintain accurate patient records using clinical systems (e.g., EMIS Web or similar). Work autonomously while collaborating with the wider team, including supervising healthcare assistants where required. Participate in audits, quality improvement projects, and infection control measures. Ensure compliance with NMC standards, practice protocols, and safeguarding procedures.

Essential Requirements

Registered Nurse (Adult) with current NMC registration. Experience in primary care/general practice nursing. Competence in chronic disease management and treatment room procedures. Strong clinical assessment and decision-making skills. Excellent communication and interpersonal abilities, with a compassionate approach to patient care. Ability to work independently and as part of a team in a fast-paced environment. IT literate, with experience of clinical systems (EMIS Web desirable). Commitment to ongoing professional development and revalidation.

Desirable

Relevant qualifications in chronic disease management (e.g., diabetes, asthma, respiratory care). Independent prescribing qualification or willingness to undertake. Experience in childhood immunisations, cervical screening, or travel health. Mentoring or teaching experience.

What We Offer
