Job Description

BAND 6 Practice Nurse required, hours and pay TBC



To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the GPs and other members of the primary care team.

An experienced practice nurse, the post holder will be a key player in delivering the primary care agenda.

Clinical

Providing nursing care and clinical services for a full range of procedures such as vaccinations/immunisation. Managing own caseload Developing and auditing standards of nursing care Running Chronic Disease Clinics, call & recall of patients and case management of these patients Providing family planning services including smears
Preventative Health and Chronic Disease Management

Managing and evaluating screening and immunisation programmes Providing health screening, planned and ad hoc, to patients of all ages and advising & taking action on the findings Managing the nursing care of all those patients with chronic disease and maintaining an appointment and recall system for patients Assisting with the implementation, review and maintenance of infection control measures in the Practice
Development, Teaching & Education

Participating in the continuing identification and assessment of the care needs of the practice population

Forming own Personal Development Plan

Participating in the process of clinical supervision

Organisation/Administration

Accountability for own clinical work - will seek guidance from the Lead Nurse/GP when clinically appropriate.
Contributing to policy development within the practice and overall practice management, providing nursing input to any decision-making processes affecting the care of patients

Initiating/contributing to the development and review of practice protocols

Maintaining accurate patient records

Maintaining equipment and supplies and ensuring correct storage of medical and surgical equipment, including vaccines

Summarising of patients records onto the clinical computer system

Liaison and Team Communications

Liaising with members of the wider Primary Health Care Team and agencies outside the practice to ensure appropriate care is provided

Participating as an active member of the Primary Health Care Team and attending meetings regularly

Meetings

It will be necessary to attend and contribute to various practice meetings as requested. The only reason for not attending will be annual, study or sick leave.
