Job Description

The Practice Nurse is responsible for delivering safe, effective, and evidence-based patient care, supporting practice policies, and promoting health and wellbeing. Core duties include supporting the Infection Prevention Control (IPC) lead through audits, training, compliance monitoring, and contributing to the annual IPC statement. The nurse develops and embeds care management programmes for frail and housebound patients, implements health promotion and wellbeing initiatives, and oversees chronic disease clinics, ensuring appropriate treatment, timely referral, and maintenance of disease registers. Key responsibilities include assessing patient needs, planning and evaluating care, diagnosing and managing acute and chronic conditions, prescribing and reviewing medications, and advising on their safe use.



The nurse co-ordinates travel medicine services, requests and processes pathology, and maintains accurate clinical records with effective coding. They support the delivery of well woman and well man clinics, implement vaccination programmes, and ensure compliance with safeguarding, chaperoning, and substance misuse policies. The role also involves assisting with minor surgery, prioritising health issues, managing clinical emergencies, recognising and referring patients with mental health needs, and liaising with external agencies to support vulnerable patients. Delegation, supervision, and safe practice within the team are essential elements.



In addition to primary responsibilities, the Practice Nurse may contribute to wider practice development. This includes developing and reviewing audit protocols and nursing protocols, supporting nursing colleagues through guidance, training, appraisals, and rota coordination, and leading service improvements. The nurse participates in local initiatives, shared learning, and policy implementation to enhance care. They review clinical practices in line with national policy, contribute to significant event analysis using structured approaches such as root cause analysis, and support the continuous improvement of protocols and service delivery.
