The successful candidate will be responsible for the delivery of practice nursing care to our practice population. They will deliver care within the boundaries and competencies of their role. Focus will be on supporting patients to live healthy lives, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively to meet the needs of patients, supporting the delivery of practice policies and procedures, as well as various Primary Care contracts such as the PCN DES, QOF and LIS.
