We are looking for an experienced and compassionate
Practice Nurse
to join our friendly, forward-thinking GP practice. You will work as a key member of our multidisciplinary team, delivering high-quality, evidence-based nursing care to our registered patient population.
This is an autonomous role with a strong focus on
chronic disease management, public health, prevention, and health promotion
, while also supporting population health management and tackling health inequalities. You will be supported by an established nursing and management team, with access to appropriate clinical supervision and ongoing development.
Key responsibilities
As a Practice Nurse, you will:
Deliver evidence-based nursing care within your scope of practice
Support long-term condition management (e.g. diabetes, asthma, COPD, hypertension)
Lead and support public health and screening programmes, including:
Immunisations and vaccinations
Cervical screening
Health promotion and prevention
Provide wound care, ECGs, venepuncture and routine nursing procedures
Support infection prevention and control (IPC) in line with policy
Contribute to QOF, enhanced services and practice targets
Maintain accurate clinical records using SNOMED CT coding
Teach and support patients to self-manage their conditions
Work with vulnerable, frail and housebound patients
Participate in clinical audits, quality improvement and governance activity
Support new-to-practice nurses and other healthcare professionals
Liaise with external services to ensure patients receive appropriate care
Support flu/COVID clinics, public health campaigns and travel advice services
Respond appropriately to clinical emergencies and incidents
What we're looking for
Essential
Registered Nurse with the NMC
Experience working as a Practice Nurse or Community Nurse
Experience of chronic disease management
Ability to work autonomously and as part of a multidisciplinary team
Strong clinical skills including immunisations, wound care and women's health
Good understanding of safeguarding adults and children
Knowledge of GP systems, QOF and enhanced services
Excellent communication, organisational and time-management skills
Commitment to ongoing professional development and revalidation
Desirable
Mentor or teaching qualification
Family planning qualification
Experience of infection prevention and control leadership
Experience supporting quality improvement or audit work
Knowledge of population health management and health inequalities
