Conduct Chronic Disease management COPD, Hypertension, Asthma and Diabetes, providing evidence-based management and patient education. Perform cervical cytology smear testsin line with national screening guidelines. Spirometry Interpretation. Support the management of long-term conditions and contribute to QOF targets.
Provide general treatment room duties, including wound care, immunisations, and health promotion. Work collaboratively with GPs, HCAs, and the wider multidisciplinary team to deliver excellent patient care. Initiating and maintaining health screening and promotion clinics for identified areas of the practice population. Deliver child immunisations and vaccinations, adult vaccinations including travel vaccinations.
Essential Requirements Registered Nurse NMC Registration required Primary Care experiencea minimum 1 year Competent in COPD and Asthma reviews Trained and competent in Cervical Cytology Excellent communication and organisational skills Wound care management. Desirable Experience with SystmOne Additional chronic disease management qualifications
