Practice Nurse or Diabetes Specialist Nurse

Key Responsibilities:

Run nurse-led clinics for

diabetes

, asthma, COPD, hypertension, and other long-term conditions Deliver high-quality

primary care nursing

services, including wound care, immunisations, cytology, and health checks Support the

GPs and multidisciplinary team

in managing chronic disease and preventive health programmes Maintain accurate patient records using

EMIS Web

About You:

Registered Nurse (NMC PIN) Experience working in a

general practice or primary care

setting Confident managing

diabetes care

and other long-term conditions Knowledge of EMIS Web preferred A proactive, patient-focused approach with excellent communication skills
