To plan and organise own workload, including participating in audit and project work.
To carry out physical examinations, investigatory procedures and cervical smear tests.
Administering immunisations, vaccinations (for example for influenza) and infant injections.
Running clinics for specific ailments such as diabetes and asthma.
Administer wound care
Administer Vaccinations (pending appropriate upskilling, qualifications and experience)
Undertake Diabetic Reviews
Maintain and develop competence and keep up to date with medical/therapeutic evidence and opinion, and local and national service, legislation and policy developments.
To develop and facilitate good working relationships with the practice staff and other community and social services.
Attend meetings of relevance.
Undertake any other duties commensurate with the post holder's grade as agreed with the clinical directors.
Delivery of specialist clinics, evidence-based practice for patients with long-term conditions, management and preventative interventions to all patients.
Referrals to social prescribers, and other community based services
Participate in vaccination and immunisation programmes for adults.
Review patient data to work with the management team, to achieve focused patient healthcare
Active participation in achieving QOF and Enhanced Services targets and recall demands
Health Checks -Blood Pressure, Height, Weight, BMI
Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being
Support patients to adopt health promotion strategies and encourage principles of
self-care
Offer holistic service to patients and their families, developing where appropriate an
on-going plan of care/support, with an emphasis on prevention and self-care.
Administration and Professional responsibilities:
To ensure own mandatory and statutory training certificates are current and in line with the organisational policy. Undertake any re-certification and patient care training as required by SS9 PCN.
Highlight any gaps in knowledge/training needs to the Practice Manager, and Nurse Manager to allow them the opportunity to offer support in order to fulfil the duties of this post
To participate in clinical supervision.
To demonstrate a clear understanding of the accountability and legal implications of the role, especially in regards to safeguarding
To participate in relevant service evaluation and improvement processes.
To participate in a regular performance review, using agreed competency framework appropriate to the individual's scope of practice.
To maintain and maximise patient confidentiality, dignity and privacy.
To comply with equal opportunities policies and procedures.
To comply with health and safety and infection control policies, including use of personal protective equipment where supplied.
To maintain a professional attitude and public image at all times.
Participate in the administrative and professional responsibilities of the practice team
Ensure accurate and legible notes of all consultations and treatments are recorded in the patient's notes
Ensure the clinical computer system is kept up to date, with accurate details recorded and amended
Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice
Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit
Attend and participate in meetings as required
Report any problems to the Practice Manager
