We are a friendly and supportive GP practice located in Sheldon. Our team is dedicated to providing the highest standard of patient care in a welcoming and compassionate environment. We are currently seeking a skilled and motivated Practice Nurse to join our team on a part-time locum basis to cover staff absence.
Key Responsibilities:
Chronic Disease Management:
Provide care for patients with chronic conditions such as asthma, diabetes, hypertension, and COPD. Offer advice on self-management and lifestyle changes.
Cervical Screening:
Conduct cervical screening (smear tests) in line with NHS guidelines.
Immunisations:
Administer routine childhood immunisations and vaccinations for both adults and children, ensuring accurate record keeping.
Health Promotion:
Provide advice and education on health promotion topics, including smoking cessation, weight management, and healthy living initiatives.
Clinical Skills:
Perform a range of practice nurse duties including blood pressure monitoring, wound care, ear syringing, and suture removal.
Spirometry and COPD Care:
Perform spirometry tests and provide management advice for patients with COPD, ensuring a high standard of care.
Diabetes Care:
Provide care and education for diabetic patients, monitor their condition, and offer guidance on lifestyle adjustments and treatment.
Record Keeping and Documentation:
Accurately document patient care and treatment in the SystmOne clinical system, adhering to confidentiality and data protection regulations.
Collaboration and Teamwork:
Work collaboratively with GPs and other members of the clinical team to ensure comprehensive patient care.
Patient Care:
Be proactive in delivering high-quality patient care, offering a compassionate and patient-centered approach to nursing.
Person Specification:
Skills & Experience:
Competent in all standard practice nurse skills including chronic disease management, child immunisations, cervical screening, and health promotion.
Experience in managing patients with chronic diseases such as asthma, COPD, and diabetes.
A good understanding of Quality and Outcomes Framework (QOF).
Experience with SystmOne clinical system is desirable but not essential.
Knowledge of spirometry and COPD management would be beneficial, with a willingness to undergo training if needed.

Qualifications:
