We are seeking a dedicated and compassionate Practice Nurse to join our healthcare team. The ideal candidate will play a vital role in providing high-quality patient care and supporting the overall health and wellbeing of our patients. As a Practice Nurse, you will work closely with doctors and other healthcare professionals to deliver excellent nursing services in a clinical setting.
Responsibilities
Deliver patient care by assessing, planning, implementing and evaluating nursing interventions.
Conduct health assessments and screenings to identify patient needs and concerns.
Administer vaccinations, medications and treatments as prescribed by physicians.
Maintain accurate patient records and documentation in compliance with healthcare regulations.
Provide education and support to patients regarding their health conditions and treatment plans.
Collaborate with the healthcare team to develop care plans that promote optimal patient outcomes.
Assist in managing chronic diseases/LTC and promoting preventive health measures.
Participate in quality improvement initiatives to enhance patient care services.
Skills
Strong knowledge of patient care practices and principles.
Excellent communication skills, both verbal and written, for effective interaction with patients and colleagues.
Ability to work collaboratively within a multidisciplinary team environment.
Strong organisational skills with attention to detail in documentation and record keeping.
Empathy and compassion towards patients, ensuring a supportive environment for their care.
