Hesa Medical Centre is seeking an experienced, dedicated, and compassionate Practice Nurse to join our friendly and dynamic primary care team. The successful candidate will play a key role in delivering high-quality patient care, supporting chronic disease management, and contributing to the overall efficiency and clinical excellence of the practice.
This role requires excellent clinical knowledge, particularly in managing long-term conditions, alongside strong EMIS Web proficiency and a good understanding of QOF (Quality and Outcomes Framework) indicators.
Key Responsibilities
Conduct comprehensive patient assessments, record medical histories, and update EMIS Web accurately.
Deliver routine nursing duties including wound care, cervical smears, vaccinations, ECGs, blood pressure monitoring, and venepuncture.
Manage chronic disease reviews and recall clinics (e.g., diabetes, asthma, COPD, hypertension).
Support the QOF and Enhanced Services workstreams by ensuring timely reviews, coding accuracy, and follow-up of patients.
Provide evidence-based health education and lifestyle advice to promote self-care and disease prevention.
Administer childhood and adult immunisations and ensure all records are maintained in line with NHS vaccination schedules.
Collaborate closely with GPs, HCAs, and the wider multidisciplinary team to deliver holistic patient care.
Support minor surgical procedures and assist clinicians as required.
Participate in audits, clinical meetings, and continuous quality improvement initiatives.
Ensure infection control, cold chain management, and clinical safety standards are consistently upheld.
Experience and Requirements
Previous experience as a Practice Nurse in a GP surgery or primary care setting (essential).
Proficiency in EMIS Web for clinical documentation, recall management, and coding (essential).
Good working knowledge of QOF indicators and experience contributing to QOF achievement targets.
Excellent communication and interpersonal skills, with a patient-centred approach.
Strong organisational and documentation skills with attention to detail.
Ability to work autonomously as well as collaboratively within a multidisciplinary team.
Up-to-date NMC registration and appropriate indemnity cover.
Desirable Skills (not essential)
Cytology training (cervical screening).
Immunisation and vaccination certification.
Chronic disease management qualification (e.g., diabetes, asthma, COPD).
Experience with NHS screening and vaccination programmes.
