Job Description

We are looking for an experienced and motivated Practice Nurse to join our friendly and supportive team at Purbeck Health Centre. You will provide safe, effective, and high-quality nursing care to our patients and play an important part in developing and improving our nursing services. Working as part of a multidisciplinary team, you will manage your own clinics and workload, deliver evidence-based care, and support service development. The role requires strong clinical knowledge, independence, and teamwork, with a commitment to maintaining professional standards and contributing to practice quality and safety.



Key Responsibilities Clinical Care Deliver high-quality, evidence-based nursing care for patients across a range of conditions. Manage chronic disease clinics, including asthma, COPD, hypertension, and diabetes (Part 1 and 2). Conduct frailty and learning disability reviews and provide care planning and monitoring. Provide womens health services, including cervical screening (must be an active smear taker), contraception, HRT reviews, and depo injections.



Deliver immunisation and vaccination programmes, including child immunisations (must be trained and confident), flu, pneumonia, shingles, whooping cough, RSV, and HPV. Undertake treatment room work, including dressings, leg ulcer management, doppler, ECGs, ear syringing, nebulising, stitch and staple removal, B12 injections, and INR testing. Carry out venepuncture, new patient medicals, wound care, and NHS Health Checks. Provide health promotion, smoking cessation, sexual health advice, and opportunistic screening.



Identify, assess, and support patients at risk of developing long-term conditions. Provide education and advice to patients about prescribed and over-the-counter medicines. Recognise, assess, and refer patients presenting with acute or mental health needs appropriately. Participate in infection prevention and control activities, audits, and compliance monitoring.



Professional and Administrative Duties Maintain accurate and detailed clinical records using SystmOne and appropriate SNOMED coding. Work within scope of practice and follow all practice and national policies. Support clinical audits, significant event reviews, and quality improvement initiatives. Contribute to the review and development of nursing policies and procedures.



Assist with minor surgery, vaccinations, and emergency response when required. Take part in clinical meetings, team discussions, and service improvement work. Support and mentor colleagues and participate in clinical supervision. Collaborative Working and Communication Work closely with GPs, nurses, healthcare assistants, pharmacists, and other professionals to ensure coordinated, safe, and effective care.



Communicate clearly and sensitively with patients, families, and colleagues. Liaise with community and external services to support vulnerable or high-need patients. Professional Conduct and Responsibilities Maintain professional registration with the NMC and comply with revalidation requirements. Work within current evidence-based practice and national guidance.



Maintain confidentiality and comply with data protection requirements. Adhere to safeguarding, infection control, health and safety, and equality policies. Take part in mandatory and role-specific training and maintain up-to-date knowledge. Support continuous improvement, learning, and reflective practice across the team.
