We are looking for an experienced and motivated Practice Nurse to join our friendly, forward-thinking team. The successful candidate will deliver high-quality nursing care as part of our multidisciplinary team, supporting our registered patient population across both practice sites. Key areas of responsibility include chronic disease management (such as asthma, COPD, diabetes, and hypertension), health promotion and disease prevention, immunisations, wound care, and well-being clinics. You will also contribute to clinical governance, helping review and implement policies and procedures to maintain safe and effective care.
This role offers the opportunity to work independently while being fully supported by a collaborative team of GPs, pharmacists, nurses, HCAs, and administrative staff. We are looking for someone with primary care experience who can make a real contribution from day one and who shares our commitment to delivering exceptional patient care.
