We are a busy GP Practice situated in Hucknall, Nottinghamshire. We deliver a full range of medical services to more than 7000 patients. Our aim is to provide an excellent and high quality service through a highly skilled, friendly and dedicated team.
We are looking to recruit a qualified and experienced Part-Time Practice Nurse to contribute to a smooth, efficient treatment room and long term condition management in a caring and efficient environment to patients ensuring that all practice needs are catered for. This role requires a proactive individual who can work independently but also support the Practice Nurse Lead and to work with the Multidisciplinary team.
Clinical Duties:
To deliver Primarily Chronic Disease Management in Diabetes, Asthma, COPD (including CHD, Hypertension, CKD, Rheumatology etc). Implement and evaluate individual treatment plans for patients with a long term condition.
Routine immunisations/Childhood Immunisations: Understanding requirements for PGD and PSD.
Travel immunisations
Cytology/Swabs
Wound care/minor injuries
Urinalysis
Venepuncture
Ear Syringing
Shared Care Prescribing Monitoring/Annual Review
Primary Prevention Clinics
