Practice Nurse Part Time
Job Description
We have an exciting opportunity for a Practice Nurse to join our team.
We are looking for a dynamic, committed and experienced individual to work collaboratively with the entire practice team, supporting the delivery of care for the practice population, including patients with acute and chronic conditions.
The successful applicant must have experience of working in general practice and will join a team consisting of a Lead Practice Nurse, Nurses, Health Care Assistants, Phlebotomists and GP Assistant.
The post holder will work as part of a friendly, vibrant multi-disciplinary team in excellent, modern purpose built premises.
You will be responsible for the delivery of Practice Nursing care to the Practice population. You will be fully supported by our experienced nurses and GP Partners within the Practice and will deliver care within the boundaries of your role, focusing upon supporting patients to be healthy. You will undertake monitoring of long-term conditions, health prevention and screening activities. You will work collaboratively with the wider Practice team working to agreed protocols in order to to meet the needs of our patients.
The candidate must have an RGN degree or equivalent and be registered with the NMC - an interest in
Diabetic care
would be an advantage.Previous Primary Care experience is essential.
