Practice Nurse - Primary Care / Long-Term Condition Management

Are you an experienced nurse with a passion for primary care and managing long-term conditions?

We're looking for a dedicated Practice Nurse to join our supportive healthcare team and deliver high-quality holistic care to our patient population.

About the Role

You will work closely with GPs, healthcare assistants and the wider practice team to provide clinical assessment, treatment and education to patients. This role is ideal for a nurse with experience in primary care or long-term condition management, including supporting patients with chronic diseases.

Key Responsibilities

Assess, diagnose and manage common minor illnesses Monitor and review patients with long-term conditions such as diabetes, asthma, COPD, hypertension and cardiovascular disease Deliver vaccinations and immunisations as required Provide health promotion, patient education and lifestyle advice Perform wound care, sexual health screening and other clinical procedures Maintain accurate clinical records and participate in audit/quality improvement activities Work collaboratively with multidisciplinary teams to ensure continuity of care

Requirements

Essential:

Registered with the Nursing and Midwifery Council (NMC) Experience in primary care or long-term condition management practice
Preferred:

Minimum of 5 years' nursing experience Strong communication & patient-centred skills
