We are seeking a dedicated Practice Nurse to join our healthcare team. The successful candidate will provide high-quality patient care within a busy medical practice, supporting doctors and other healthcare professionals. The Practice Nurse will be responsible for delivering comprehensive nursing services, ensuring patient comfort and safety, and maintaining accurate medical records.
Main duties and responsibilities include:
a) Chronic disease management in line with QOF Targets- Diabetes, Asthma & COPD
b) Wound management including dressings
c) Child and adult immunisations
d) Screening- Including Cervical Screening
e) Health promotion
f) Ear care
g) Child health/child protection
h) Performing ECGs
i) Venepuncture and requesting pathology testing
j) Uphold Infection control standards and ensure clinical environment is kept clean and stocked.
k) Chaperoning and assisting patients
l) Attend and participate in clinical and practice meetings when required
m) Proactive in identifying own needs for personal and professional development
n) Ensure all duties are carried out to the highest possible standard and in accordance with current clinical and practice guidelines and initiatives.
o) Provide patient care in accordance with current PGDs and clinical protocols
p) Ensure patient care activities are carried out in line with QOF requirements.

Person Specification Essential:
Registered with the NMC (Nursing & Midwifery Council)
Post Graduate diploma or degree
Experience of working in primary care
Experience of chronic disease management (Asthma, COPD, Diabetes)
Experience of working autonomously
Knowledge of and ability to work to policies and procedures
Disclosure Barring Service (DBS) check
Evidence of training qualifications

Full UK Driving License
