We are a busy, friendly, and expanding three-partner practice, currently caring for a population of approximately 16,000 patients across our dispensing sites. To support our growing services, we are looking to recruit 2 dedicated
Practice Nurses
to join our experienced clinical team, working flexibly across our sites.
About the Role:
We are ideally seeking someone to work 2
0-40 hours per week
, with experience in
chronic disease management
--particularly
diabetes
, within
general practice.
The successful candidate will be:
A registered nurse (NMC required)
A team player who is reliable, well-organised, and a strong communicator
Able to work efficiently under pressure and engage confidently with a diverse patient population
Key Responsibilities:
Cervical cytology
Management of chronic diseases
Delivering care in line with local and national guidelines
Familiarity with QOF targets and documentation
Working within the framework of Locally Commissioned Services and Direct Enhanced Services
Participation in regular clinical and practice meetings
Systems used:
EMIS Web, INR Star, Accurx and Docman (training provided if required)
