Prescription Clerk

Job Summary

The purpose of the role is to:

Manage the repeat prescription process Offer prescription related assistance to the GP's, Clinical Pharmacists, Nursing Team, Reception Teams, Pharmacies, and other Health Care Professionals Answer prescription related queries from patients as per Protocols and guidelines Undertake a variety of administrative duties to assist in the smooth running of the Prescription Process within the Practice. It is the duty of each team member to assist in the smooth running of the Practice in general. It is also the responsibility of each employee to make sure that whoever encounters the practice, either in person or via the telephone, receives the highest standard of respect, courtesy, fairness, and professionalism

Duties:



Process electronic prescription from online, email or written request from patients,

pharmacies, District Nurses and Appliance contractors

Work through SystmOnline online prescriptions requests, prescription related

tasks, scans/workflow folder and emails.

Adhere to set prescription protocols for checking high risk or monitored medication

Check for MCS (Manufacturer Cannot Supply) medication and keep up to date on alternative

medication available at pharmacies

Work closely with Clinical Pharmacist and GPs to give high quality medication care to patients

Process blood request on ICE system inline with medication review dates. Ensure reviews are

booked in yearly with patients on mediation

Have full understanding of SystmOne, Word, Outlook ICE, EPS Tracker.

Add, remove, and alter meds on repeat or acute lists according to clinician/hospital letters

requests within agreed competency framework.

Be aware of vulnerable and monitored patients for added patient care.

Identify patients with medicine adherence concerns.

Identify potential eRD patients in line with CCG information. Set up eRD and maintain eligible patients ongoing.

Deal with prescription related telephone queries/requests from patients and act as liaison with the Doctors or Clinical Pharmacists as necessary.

Deal with requests from patients to resynchronise their medication and amend medicine quantities as needed.

Develop close working links with local Community Pharmacies.

Arrange blister packs set up on request.

Assist the Practice in meeting its Quality Outcome Framework targets

Accessing the Practice appointments system to book medication reviews or other prescription related issues

Ensure that requests for repeat prescriptions are generated accurately and efficiently,

following practice protocol and authorisation by prescriber, within two working days specified by the practice and in accordance with Practice guidelines.

Always dispose of documents as appropriate observing confidentiality and data protection.
Ensure all patient information not required is shredded.

Action tasks as requested by clinical staff via SystmOne and email. Daily check of email and act upon them.
As per both Government legislation and Practice policies ensure that all confidentiality, data protection and information governance policies and guidelines are followed and strictly adhered to.

The post-holder will participate in any training programme implemented by the Practice as part of this employment, such training to include:

To work flexibly and effectively within the Practice teams so that essential priority tasks are covered and dealt with.

Other duties as may be required in accordance with this grade of post.

Personal Specification

Experience

Essential

Experience of dealing with public. Understanding of confidentiality and disclosing information Knowledge and use of Microsoft work Ability to work under pressure, worked with data bases.

Desirable

Experience of working in a GP practice Knowledge of patient administration systems worked as an receptionist/administrator.

Qualifications

Qualifications GCSE grades in English & Maths Qualified to NVQ level 2 / 3.
