The Prescribing Pharmacist works as part of the multidisciplinary primary care team to provide expert medicines management, structured medication reviews, long-term condition support, and clinical assessment for patients. The postholder will use their prescribing qualification to optimise treatments, support safe and effective prescribing, and improve clinical outcomes across the practice.
Key Responsibilities
Clinical Responsibilities
Conduct structured medication reviews for patients with complex polypharmacy, frailty, or multiple long-term conditions.
Independently prescribe, deprescribe, and optimise medications in line with evidence-based guidelines.
Support the management of long-term conditions such as hypertension, asthma/COPD, diabetes, heart failure, and cardiovascular disease (depending on experience).
Assess and manage patients presenting with medicines-related issues and minor or routine clinical presentations within scope of practice.
Provide medication counselling, adherence support, and lifestyle advice to patients.
Conduct medication audits, safety reviews, and contribute to quality improvement activities.
Support repeat prescription processes to ensure safety, efficiency, and compliance with practice protocols.
Reconcile medicines for patients following hospital discharge, outpatient appointments, or care transitions.
Identify high-risk prescribing and implement interventions to reduce patient risk.
Request and interpret relevant tests (e.g., BP, spirometry results, blood tests) to support clinical decision-making.
Multidisciplinary & Operational Duties
Work closely with GPs, ANPs, paramedics, nurses, and the wider team to deliver coordinated patient care.
Contribute to QOF, IIF, and other national/local service requirements.
Participate in MDT meetings and patient case discussions, including frailty and complex care reviews.
Provide medicines-related training or support to clinicians, administrative staff, and patients.
Support antimicrobial stewardship initiatives.
Ensure all clinical work is accurately documented in patient records.
Qualifications & Requirements
Essential
GPhC-registered Pharmacist.
Independent Prescribing qualification (or working towards completion).
Experience in primary care, community pharmacy, PCN roles, or hospital clinical pharmacy.
Strong clinical knowledge and understanding of medicines optimisation principles.
Excellent communication, organisation, and problem-solving skills.
Ability to work autonomously and as part of an MDT.
Desirable
Experience using EMIS Web or other primary care clinical systems.
Training/experience in long-term condition management.
Experience supporting QOF, audits, or quality improvement projects.
Experience working within a PCN, general practice, care homes, or frailty services.
