JOB SUMMARY



Salary:

53,000 (Negotiable depending on experience)

Willesden Green Surgery is seeking to recruit a full time (37.5 hours a week) or part time

prescribing pharmacist

to see and treat patients with a wide range of ailments.

Willesden Green Surgery is a GP practice located in Brent, London, renowned for its rapid growth and diversity. It is highly regarded by its patients, with the practice consistently rated as one of the top three GP practices in Brent according to the National GP Patient Survey. Comprising a team of six highly skilled and supportive GP partners and four excellent salaried GP, each with their own unique areas of expertise, the practice ensures comprehensive and specialised care for its patients. With a commitment to expansion and innovation, Willesden Green Surgery is actively pursuing the establishment of a state-of-the-art multipurpose centre that will provide a wide range of amenities for both patients and staff, further enhancing the quality and accessibility of healthcare services for the community. Due to it's rapid exapansion, the surgery has secured a second site in Queen's Park (William Dunbar House), which will provide more space for our patients to be seen.

In this role, the clinical pharmacist will be given daily support by the GP partners who will provide training, mentorship and supervision.

The clinical pharmacist will need to be committed to improving health and putting patients first. Key qualities will be the ability to build effective relationships with patients, GPs, nurses, community pharmacists, hospital & practice staff, and to work effectively as part of a team. Demonstrable general pharmaceutical skills and knowledge in core areas, including long term conditions, together with a good understanding of general practice are essential.

The post holder will work within the Anson Road and William Dunbar House sites serving a population of 15,000 patients.

The post holder will work as part of a multi-disciplinary team to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas, while also helping to address both the public health and social care needs of patients and help in tackling inequalities.

The post holder will review patients with chronic diseases and undertake clinical medication reviews, using a person-centred approach to consultation, involving patients in decisions about their medicines and identify motivating factors to improve adherence. Pharmacists will focus on patients with multiple long-term conditions on complex polypharmacy.

The post holder will have a key role in supporting delivery of the new network contract Directly Enhanced Service (DES) specifications as well as the already established Quality Outcomes Framework (QOF) specifications.

The post holder will provide support to general practice staff with prescription and medication queries, dealing with acute prescription requests and medicines reconciliation on transfer of care.

The post holder will work towards eventually being able assess and manage patients who present with a wide range of ailments.

DETAILED ASPECTS OF THE ROLE



Undertaking medications reviews:

Review patients with single or multiple medical problems where medicine optimisation is required. Review on-going need for each medicine, assess monitoring requirements and support patients to ensure treatment is safe, appropriate and optimised for maximum clinical benefit. Discuss recommendations and outcomes with relevant members of the team and provide appropriate and timely follow up. Support patients to optimise medicines by providing additional written patient information leaflets, and liaise with community pharmacists about the need for special requirements to aid adherence.
Repeat Prescribing:

Review, implement and support adherence to the practice repeat prescribing policy. Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates and flagging up those patients needing a review. Syncing and standardising patients' medications i.e. all medicines to be requested at the same time on 2 monthly prescription (except controlled drugs which should be one monthly) Ensure patients on repeat prescriptions have the required monitoring and assessments undertaken as appropriate to their condition(s) and medicines prescribed e.g. blood test, blood pressure, asthma and COPD reviews Reducing wastage and optimisation of medicines across the practice DMD corrections to facilitate EPS
Patient medication queries:

Responsible for handling medication queries from patients including acute prescription requests, early requests, controlled drug requests, whilst differentiating when a query can and cannot be resolved by a principal GP. Provide a telephone help line for patients with questions, queries and concerns about their medicines. Possess comprehensive knowledge of pharmaceuticals, drug interactions, dosage regimens, and common side effects. Effectively and clearly explain medication-related information to patients in a manner that is easy to understand.
Medicines Reconciliation:

Reconcile medicines following discharge from hospitals, or transfer from other practices. Identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge or following transfer from other practices.
Delivery of Quality Outcomes Framework (QOF) targets:

Delivery of prescribing QOF targets including atrial fibrillation, asthma, COPD, diabetes, CKD, heart failure, hypertension, coronary heart disease, stroke, mental health, rheumatoid arthritis and drug dependency targets
Delivery of PCN DES and IIF targets:

Delivery of prescribing PCN IIF targets including hypertension, cardiovascular disease prevention, atrial fibrillation, structured medication reviews for high risk groups, asthma. Delivery of prescribing PCN DES targets including hypertension, cardiovascular disease prevention, and medicines optimisation targets.
Management of Minor Ailments:

Support practices in improving patient access for minor ailments and acute presentations. Promote the importance of self-care and signpost patients to community pharmacies in line with national guidance on over the counter medicines.
Managment of Major or Chronic Health Conditions

Work towards being able to take a history, examine, investigate, diagnose and treatment major or chronic health conditions under GP supervision
Medicines Information to Practice Staff and Patients:

Support relevant medicine-related enquiries from the practice team, community pharmacists and patients. Update practice teams at weekly meetings on current medicines related information and issues with a view to educating and improving prescribing within the practice. Analyse, interpret and present medicines data to practice staff regularly to highlight prescribing issues. Communicate recommended actions relating to medication and medical device safety alerts.
Signposting:

Ensure that patients are referred to the appropriate healthcare professional for specialist referral, monitoring and/or referred to the social prescriber for further support services where required. Booking patients in to speak to a GP when a concern or query is beyond their scope
Clinical Governance:

Communicate, action and support implementation of MHRA alerts, product withdrawals and other local and national guidelines and formulary recommendations. Undertake clinical audits in priority areas identified by the Northwest London ICS medicines management team or within the PCN and feedback results to the practice and PCN and support them in implementing recommendations. Support implementation of shared care protocols in practice, including processes to improve monitoring of high-risk drugs. Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved.
Education and Training:

Contribute to the provision of education and training on medicines optimisation and specific therapeutic areas to healthcare professionals within the practice.

KEY COLLABORATIVE WORKING RELATIONSHIPS



The post-holder will be required to work with a wide range of colleagues within the practice. It is important that the post holder recognises the roles of other colleagues to person-centred care, and liaises with other colleagues as needed for the collective benefit of patients including:

Patients GPs, nurses and other reception staff Other external healthcare professionals including community pharmacists, pharmacy technicians, community nurses, optometrists, dentists, health and social care teams and dieticians Ability to work as a member of a team within the PCN

KNOWLEDGE, SKILLS AND EXPERIENCE REQUIRED



Has experience/awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice. Minimum of 1 year post graduate experience in General Practice, as demonstrated within a practice portfolio. Holds an independent prescribing qualification. Demonstrates an understanding of the role of a clinical pharmacist within primary care and where it fits into and how it complements the practice team and the wider PCN team. Able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct.
