Maritime Health Partnership is a leading Primary Care provider operating a network of GP Surgeries across Medway and Maidstone localities. We are known for providing high quality Primary Care service and are now looking for an experienced individual to join our prescription hub.
The successful candidates will work as part of a dedicated Prescribing Hub team taking responsibility for the accurate and timely processing of repeat prescription requests on behalf of registered patients of Maritime Health Partnership.
Duties may include but are not limited to, the processing of repeat prescriptions in electronic and hard copy format, including online requests. The post-holder will support the Hub team as part of a shared responsibility for handling incoming prescription queries via telephone, encouraging uptake of the NHS App and use of Voice Connect.
The role will require the post holder to ensure that medication is issued firmly in line with the practice safe prescribing policy and that every contact with our patients counts towards a quality outcome and good patient experience.
You will be the main point of contact for Patient enquiries about their prescriptions, so resilience is important. You will be liaising on sensitive medical information, so a caring customer service communication style is essential.
This role requires you to use your exceptional organisational and prioritising skills, combined with top level attention to detail.
Other ad-hoc Administrative and Reception duties may be required to support the practice.
Some of the Main Responsibility and Key Tasks;
Deal with prescription queries from patients, care homes and community pharmacies.
To process repeat prescriptions within a timely scale to the highest standard
To manage the prescribing process including all requests, monitoring usage and informing the Hub Lead of any compliance issues or queries and escalating concerns or enquiries to the Consultant Pharmacist
Supporting patients to manage their medicines, for example, synchronisation of medications, reviewing patient's suitability for monitored dosage systems, and setting up electronic repeat dispensing.
Seek advice from Hub Lead and escalate enquiries out of scope of competences to clinical lead as appropriate.
Contribute to the development of efficient and effective services and ensure good patient experience.
Support the achievement of the practices prescribing targets i.e. QOF, IIF and local incentive scheme.
To support the safe and effective repeat prescribing of high risk medicines, ensuring that regular monitoring as per local/shared care guidance is taking place.
To communicate effectively with patients and their carers ensuring they understand the medication they are on, the importance of compliance including medication review and why tests have been requested
Recognise people's needs for alternative methods of communication and respond accordingly.
To communicate effectively with other team members and act as an ambassador for the Hub service and team.
To support patients that are frail and elderly and those that are vulnerable to achieve ease of safe access to appropriate medication in a timely way.
To liaise with community pharmacies and care homes if any issues arise.
To cover late reception as part of a wider staff rota working arrangement.
