We are looking to recruit a clinical pharmacist interested in developing their career in primary care, improving patient outcomes, reducing prescribing variation and ensuring patient safety. With the increasing burden of polypharmacy and complex co-morbidities, we are looking for a pharmacist with a keen understanding of balancing risks and benefits in relation to medicines and patient circumstances.



The position involves working 4 days within WISE 3 PCN; at Heathcot Medical Centre in Woking and one day at St. Peter's Hospital.



You will be motivated, and passionate about patient care with excellent communication skills to work with a multi-disciplinary team across the locality to optimise medications with a patient-centred approach. This cross-sector roll allows the post holder to understand and develop services in both primary and secondary care, to enhance patient experience and cross-sector collaboration. It will involve undertaking work to help prevent hospital admissions and improve medicines use across the interface.



This pharmacist post is a great opportunity for pharmacists wishing to develop their career. You will ideally be a prescriber with experience of working within hospital or primary care.

The post holder will be responsible for:

Support delivery of medicines related QoF and CQC indicators, local prescribing incentive scheme and medicines safety agenda

Support the practice with chronic disease management and day-to-day medicines related queries

Promoting cost effective and evidence based prescribing, following national and local guidelines.

Supporting the delivery of the pharmacy and medicines optimisation elements of the network service specifications within the Network Contract DES with a particular focus on the structured medications review and optimisation specification and the enhanced health in care homes specification.



Support medicines optimisation across the interface and within the PCN

Improve medicines safety and patient outcomes relating to medicines.

Support in the management of chronic diseases, antimicrobial stewardship and polypharmacy

Promote cross-sector collaboration and support with relevant interface projects

Review and support the development of relevant policies and procedures

To support the networking, education and development of trainee and foundation pharmacist



Undertaking further education required for the role including CPPE Primary Care Pathway and Independent prescribing.

t The position is current based at Heathcot Medical Centre for 4 days and 1 day at St. Peter's Hospital. WISE 3 PCN includes 3 GP practices each with multiple sites, the post holder may have to work across these site should the need arise.

1. MAIN DUTIES AND RESPONSIBILITIES OF THE POST:

Support the delivery of the medicines related QoF and CQC indicators, local prescribing incentive scheme and medicines safety agenda

Support the practice with chronic disease management and day-to-day medicines related queries

Promoting cost effective and evidence-based prescribing, following national and local prescribing and safety guidelines.

Supporting the delivery of the pharmacy and medicines optimisation elements of the network service specifications within the Network Contract DES with a particular focus on the structured medications review and optimisation specification and the enhanced health in care homes specification.

Improve use of medicines across the system, this may include but not be limited to:



Inappropriate use of antibiotics

Polypharmacy

Medicines optimisation in chronic diseases such as Asthma / COPD and cardiovascular disease

Medicines at risk of misuse

Care Home residents

Management of medicines after discharge from hospital, reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and manages these changes without referral to a GP

Medicines Safety

Reducing waste and sustainability

Cost effective and evidence base prescribing.



Support medicines optimisation across the interface and within the PCN

To participate in specified interface project/audits, according to need that supports medicines optimisation activities

Promote cross-sector collaboration and support with medicines related interface projects

Review and support the development of relevant policies and procedures

Management of medicines after discharge from hospital, reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and manages these changes

Work in partnership with secondary care colleagues to proactively manage patients at high risk of medicine related problems.

Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individualised patient reviews.

Put in place changes to reduce the prescribing of these medicines to high risk patient groups.

To support the networking, education and development of trainee and foundation pharmacist / technicians

Work in partnership with secondary care colleagues to proactively manage patients at high risk of medicine related problems before discharge to ensure continuity of care.



Collaborate with pharmacy colleagues across the different sectors to ensure medicines optimisation across the interface.

Supporting further integration of general practice with the wide healthcare teams (including community and hospital pharmacy) to help improve patient outcomes

Manage the clinical aspects of shared care protocols, clinical research with medicines and, liaison with specialist pharmacists.



Undertaking further education required for the role including CPPE Primary Care Pathway and Independent prescribing.

To maintain registration, as a pharmacist, with the General Pharmaceutical Council, and to act in accordance with the code of ethics.

The post holder agrees to participate in Continuing Professional Development (CPD) including further training and learning in order to develop the knowledge, understanding, skills, attitudes and values to become competent in the duties described. To undertake peer review and competency-based assessment to ensure required standards of service are maintained.

Be involved in clinical pharmacy training/teaching to pre-registration pharmacists, technicians, ATOs and junior pharmacists.

Provide pharmaceutical education and training to other healthcare staff in all areas to which you have a responsibility.

To follow all Health and Safety standards in compliance with Trust and PCN policies and procedures. This includes:

Report all accidents, incidents and near misses

Dealing with hazardous chemicals (including cytotoxic drugs) used in the department, following COSHH regulations.

Develop, review and follow all standard operating procedures (SOPs) accurately.

Maintain accurate manual and computer records.

Your hours are based on a whole time week of 37.5 hours. Hours are calculated excluding meal breaks and are to be worked in accordance with the needs of the service and as agreed with your manager. The Trust operates a 24 hour 365 day per year service. The way in which the contracted hours are worked may change in accordance with the needs of the service, following consultation
