We are looking to recruit a clinical pharmacist interested in developing their career in primary care, improving patient outcomes, reducing prescribing variation and ensuring patient safety. With the increasing burden of polypharmacy and complex co-morbidities, we are looking for a pharmacist with a keen understanding of balancing risks and benefits in relation to medicines and patient circumstances.
The position involves working 4 days within WISE 3 PCN; at Heathcot Medical Centre in Woking and one day at St. Peter's Hospital.
You will be motivated, and passionate about patient care with excellent communication skills to work with a multi-disciplinary team across the locality to optimise medications with a patient-centred approach. This cross-sector roll allows the post holder to understand and develop services in both primary and secondary care, to enhance patient experience and cross-sector collaboration. It will involve undertaking work to help prevent hospital admissions and improve medicines use across the interface.
