Job Description

Providing a range of Medicines Management services to our patients, including the completion of Medicines Reconciliation, completing discharge prescriptions at ward level and counselling patients on new medications Act as a role model and mentor to junior staff, providing support, guidance and training as needed in order to develop staff. Embedding yourself as a key member of the ward team, supporting the MDT towards outstanding patient outcomes. Identify and contribute to service improvement, evaluation and development.
