Scope of the job



Tyneside and Northumberland Mind are looking to recruit an effective, highly motivated, and confident project coordinator to join their highly skilled team. The successful candidate will be part of a dynamic team working within our Northumberland and Tyneside offices.

Working as part of Tyneside and Northumberland Mind's counselling service, the

Project Coordinator

, will act as the primary point of contact for health care professionals, clients and counsellors, providing critical operational support and overseeing daily logistics. Arranging and coordinating counselling sessions for children and young people affected by suicide and/or trauma across the North East and Cumbria. Acting as a central point of contact to ensure appropriate support is made available to them, with a requirement to develop an in-depth knowledge of the local health and care infrastructure

The role is client facing providing triage support, arranging assessments and managing the Case Record Management system. You will contribute to the monitoring and evaluation of this new project. Working with the Trauma Intervention Service Manager and other agencies to support and further develop the service that will demonstrate the impact and value the charity has.

This is an exciting opportunity for a proactive individual with experience of working with vulnerable children and young people.

As a key member of the Tyneside and Northumberland Mind team you may deal with telephone enquiries which may sometimes prove to be challenging or distressing due to the nature of the clients we support.

Although predominantly office based the role requires a willingness to work flexible hours when required to meet work demands, including potential on-call availability and out of hours working. Also, the occasional requirement for travel in working hours within the North East and Cumbria region.

Key Responsibilities



Call, referral and email management, maintenance of the CRM system (Charity Log), keeping accurate and up-to-date records.

Reporting from CRM systems to support the organisation.

Arranging telephone assessment appointments.

Signposting to services.

Escalating complaints and compliments.

Communicating with other services to support our clients.

Day to day management of our support line service.

To work within the counselling service, supporting the Project Manager.

Oversee high-quality person-centred care, maintaining strong relationships with clients, families and stakeholders.

Ability to work both independently and as part of a team, with a calm approach under pressure.

Develop an in-depth knowledge of the local health and care infrastructure.

Identify when action or additional support is needed, alerting a named clinical contact, highlighting any safety concerns. (Safeguarding).

Adhering to information governance and data protection legislation.

Work sensitively with people, their families and carers to capture key information.

Encourage people, their families and carers to provide feedback and to share their stories about the impact of the service on their lives.

Record and collate information according to agreed protocols and contribute to evaluation reports required for the monitoring and quality improvement of the service.

Skills



Experience of working directly in a care coordinator role, adult health and social care, learning support or public health / health improvement environment.

Experience of working in health, social care and other support roles in direct contact with people, families or carers.

Professional telephone manner and empathy for vulnerable clients

Working effectively as part of a team, flexible attitude, proactive task management

Experience and evidence of providing quality customer service.

To be sensitive to and aware of issues relating to client confidentiality and experience working within a confidential client service.

Commitment to Tyneside and Northumberland Mind's Vision, Mission and Values.

Experience of working in an office environment.

Experience of working with and competent in the use of, Microsoft Office specifically Word, Outlook and Excel.

Experience of producing documents, such as, forms, minutes, agendas, reports.

Comfortable engaging with people who have mental ill health or who may be experiencing emotional distress.

Ability to actively listen, empathise with people and provide personalised support in a non-judgemental way.

Commitment to reducing health inequalities and proactively working to reach people from diverse communities.

Ability to support people in a way that inspires trust and confidence, motivating others to reach their potential.

Ability to identify risk and assess / manage risk when working with individuals.

Have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when what the person needs is beyond the scope of the care coordinator role - e.g. when there is a mental health need requiring a qualified practitioner.

Knowledge of Safeguarding Children and Vulnerable Adults policies and processes.

Ability to maintain effective working relationships and to promote collaborative practice with all colleagues.

Ability to demonstrate personal accountability, emotional resilience and work well under pressure.

Ability to organise, plan and prioritise on own initiative, including when under pressure and meeting deadlines.

Experience of data collection and using tools to measure the impact of services.

Desirable



Experience of working within or knowledge of mental health services.

Experience of working on Case Record Management systems.

Experience of working within multiprofessional team environments.

Conditions of employment



In the light of legislation [Section 8 Asylum and Immigration Act, January 1997], as a condition of being able to commence employment, the successful candidate will be required to provide documentation to prove that they are able to work in the UK. Probation



The probation period of this role is 3 months.

Expenses



Out of pocket expenses and travel will be payable at rates set out in Tyneside and Northumberland Mind's expenses policy.

Holidays



On joining, pro rata on a 24 working days per annum (plus Bank Holidays). Thereafter, the holiday entitlement increases depending on length of service. (The holiday year runs from 1 April to 31 March.)
