Grade: G4, 26,403 - 28,142 Pro Rata to hours worked


Working 30 hours per week



An exciting opportunity has arisen to join Intermediate Care Service in Walsall, this is a multi-disciplinary team operated jointly between Walsall Healthcare NHS Trust and Walsall Adult Social Care.


We are particularly looking for enthusiastic and committed individuals to join our team to help shape and deliver our services, and contribute to the model of best practice. The successful candidates will be required to have experience of working in Adult Social Care and to join our Multi-Disciplinary Team.


You will be required to support customers in their own homes to develop or regain skills and confidence, to co-ordinate and deliver a programme of active Reablement to support customers to achieve their full potential, working in accordance with individualised Reablement Plans. To work cohesively within a multidisciplinary team to maximise customer independence and reduce the need for ongoing support. To support customers to enjoy full access to the resources in their community, develop informal support networks.

About the role:



As part of this role,


To support/enable activities of daily living (ADL's) including personal care, (as part of a responsive service in partnership with other agencies) as required as part of an overall support plan, ensuring that religious, cultural and gender needs are met, and personal dignity and privacy are maintained. All care interventions will be agreed with the service user and written within a support plan. The Reablement Officer will at all times support/enable activities of daily living (ADL's) within the agreed parameters set out in the persons support plan. . Minimum qualification for this role NVQ L2 Health and social care or equivalent.
