Experienced Rehabilitation Case Manager

Are you a nurse, OT or physio with case management experience? Do you want to work from home as part of a small team of supportive healthcare professionals?

: Yorkshire/North East, Midlands and South West

Working remotely or from our offices in Pocklington or Darlington

Hours

: 37.5 hours per week between Mon to Fri 8.30am to 5:00pm

Status

: Permanent employed contract

Salary

: 43,000 - 50,000 inclusive of 3000 car allowance dependent on experience of personal injury case management.

Qualifications, Skills and Experience

: Five years of post-registration (NMC or HCPC) clinical work experience. For higher salary bracket minimum two years of Case Management work experience within

personal injury claims following 5 years post registration clinical practice.

Bridge Case Management is a family-run, friendly team of case managers, working with clients and the litigating parties involved in personal injury claims. We specialise in cases that usually involve orthopaedic injuries and/or amputation, psychological issues, and pain. Many of these are complex. We work with the lawyers and insurers who are involved as a result of our clients' personal injury claims. We create unique multidisciplinary teams for each client, acting as a "project manager" for those involved in the rehabilitation plan, while liaising with the litigating parties to ensure funds are available.

For applicants with no experience in this area, we will provide a full training programme.

To achieve the higher rate salary, we are looking for a case manager with experience of working within personal injury law.

We are looking for new team members with a passion for supporting people to achieve optimum rehabilitation outcomes following traumatic injuries.

A vital part of the case manager role is management of agreed budgets and making appropriate, cost-effective recommendations as part of the rehabilitation plan for each client. Therefore, we are looking for a case manager who can demonstrate experience and confidence in budget-management.

At Bridge Case Management we prioritise training and clinical supervision of our team members. Even through the pandemic and the lock downs we've maintained a strong team and will always make sure that help and advice is at hand.

Salary:



43,000 - 50,000 dependent on level of case management experience in personal injury

The successful candidate will:



Be an excellent communicator, maintaining relationships with clients and those involved in their rehabilitation via phone, video calls and in person Have flexible thinking and problem-solving skills Have excellent time management and administration skills Have excellent written skills - report writing and communicating effectively with all involved Can make clinically sound recommendations for intervention Have confidence and ability to convince and influence stakeholders by sound clinical judgement and reasoning Meet clients in their homes and to clinical appointments, own transport required Own car which must be insured for business use

Experience and Qualifications:



Essential:



Qualified nurse, occupational therapist or physiotherapist (5 years post-registration experience) UK registration of NMC or HCPC Working from within the UK, ideally with experience of work within the NHS Driving licence and car available for client visits

Preferred:



Two years case management experience in rehabilitation from personal injury An understanding of statutory services
