Our organization is looking for highly experienced rehabilitation case managers to join our team on a self-employed basis. The case manager will be responsible for completing immediate needs assessments and providing ongoing case management services to clients who have sustained brain injuries, spinal injuries and orthopaedic injuries.
Responsibilities:
Be a registered healthcare or social work professional eg. Physiotherapist, Occupational Therapist, Social Worker, Nurse Have a minimum of 5 years' experience as a complex case manager. Be able to provide robust rehabilitative assessments and treatment plans for our seriously injured clients. To work in alignment with our values of compassion, support and integrity and always providing a client focused service. Have a proven track record in setting up and managing complex packages of rehabilitation and care within the community. Work collaboratively with all stakeholders including other healthcare professionals, clients and their families, legal teams and insurers.
Job Type: Freelance
Pay: 70.00-75.00 per hour
