Job Description

This position would suit individuals who have had at least three years care home experience and have knowledge of care planning. Full support and training will be provided to help candidates settle into the position.

As Rehabilitation Co-ordinator you will be working in a small residential home for adults with acquired brain injuries. Full training and support will be given to the successful candidate. The job entails working as a care support worker in an enhanced role which requires you to build and develop care and rehabilitation plans with the support of the Home Manager and Quality and Rehabilitation Systems Manager. Hours worked will be according to a rota which includes weekend working. You will be part of a team providing direct personal care and rehabilitation to clients in the home. You will receive on-going training, guidance, and support. Your work will require you to champion and actively maintain the systematic approach to care and rehabilitation adopted by the organisation. This will require the collaborative setting and monitoring of client's goal plans with a view to always seeking progress and positive outcomes. You will construct client goals through continuous assessment using the home's assessment documentation and liaising with the Quality and Rehabilitation Systems Manager (QRSM), Home Manager, Senior Care/ Team Leader, and client. Your role will require you to organise your time effectively, putting dedicated time aside to carry out your role specific activities which support the systematic approach to care and rehabilitation adopted by the organisation. You will receive help with this by the Home Manager, Senior Care Team Leader and the QRSM. As with all staff, you are required to vigilantly seek out all risk assessments. Your role requires requires you to participate in, and develop, reactive management plans regarding aspects of health and safety and client behaviour, ensuring that they can be implemented effectively

You must be able to drive and maintain a current UK driving licence. Your role will demand working with other professional disciplines, research, organising meetings and carrying out assessments. At all times you must maintain a professional outlook and effectively realise your role as ambassador for the organisation. Your position will require regular training/ education which you must undertake at the discretion of the Employer. You should actively improve your knowledge base/skills independently and are obliged to attend compulsory monthly support meetings with the Home Manager to address your performance and other work-related issues.

In association with the above, the role expectancies for this position are to:

Work closely with the Quality and Rehabilitation Systems Manager, Home Manager, Deputy Home Manager, care support staff who are named carers for individual clients and clients to devise care and rehabilitation plans. Complete and keep up-to-date client assessments for all clients in the Home. Routinely liaise with the Quality and Rehabilitation Systems Manager in the development of client's assessments and care and rehabilitation goal plans. Continually evaluate and progress client's goal plans. Write a report based on the up-to-date assessment in time for each client review. Manage your time effectively to ensure that you are able to maintain your role effectively. Work directly with clients in the implementation and monitoring of goal plans. Undertake direct care activities including personal care as part of the staff team, whilst maintaining your role through effective time management. Ensure that the Home Manager and Deputy Home Manager are fully informed and kept up to date with the care and rehabilitation plans and with the rationale behind them. Collaborate with clients and their named care support staff in prioritising and development of care and rehabilitation plans. Liaise with other health and social care professionals as required in the performance of your role as Rehabilitation Coordinator. Organise client reviews, completing the agenda and inviting pertinent others. Complete the Individual Program Plan (IPP) for each client following each review in a timely way, ensuring that IPP outcomes are completed as the IPP period progresses. Research the environment in order to identify suitable resources to support clients' care and rehabilitation plans. Maintain and further develop your knowledge and skills pertinent to your role through active research. Maintain a safe and healthy environment for all clients. Work to establish a comfortable, homely environment for all clients. Work within the statutory regulations and conditions relating to Residential Homes. Maintain a first-aid, food hygiene and manual handling certificate. Maintain an up-to-date knowledge of the safeguarding of vulnerable adults. Administer medication to clients in line with medication training directives and the home's policy and procedure. Respect the rights, privacy, and dignity of all clients with whom you work. Build a network of resources for clients. Drive and hold a full, clean, current UK driving license and be able to drive the vehicle/s attached to the home. Actively maintain awareness of current prOKare policies and procedures and adhere to them. Undertake training/ education at the discretion of the Employer. Effectively use a computer to record information and undertake searches. You should actively improve your knowledge base/skills independently. You are obliged to attend compulsory monthly support meetings with the Home Manager to address your performance and other work-related issues. Ensure that you maintain full awareness of the management and operational systems that are being implemented in the home and can demonstrate understanding of them.

This will depend on the circumstances of your offence. Criminal convictions and associated information will be discussed with applicants during selection.

Rehabilitation Coordinators work a 35-hour week based on three non-consecutive days from 8.20am - 9.10pm with two x 40-minute breaks. On occasions, and only where necessary to meet the specific demands of the home, other hours may be agreed on a temporary basis, with the Home Manager. Where this occurs, the agreed hours will not constitute a change to your contracted hours of three non-consecutive days 8.20am - 9.10pm and should be reviewed weekly. Any changes should be clearly indicated on the Home's rota. All staff are contracted to attend a 2hr 10min staff training/meeting session each calendar month.

The rate of pay is 12.90 hour (with continuity bonus equivalent 13.23 hour)*. The first six months of employment are considered a probationary period. During this period long term employment in this role is not guaranteed and will be dependent on performance and the effectiveness of the role.

of the following month you work without a break for sickness/absence or lateness of one hour or more, or frustration of the Home's rota by withdrawing your services (as in the case of overtime/ time-in-lieu being agreed) less than 3 days before such shift should commence and for maintaining attendance at compulsory monthly in-service education and at compulsory training events specified by the company at its discretion, which are considered necessary for your employment.

You will be paid in arrears, at monthly intervals for the working hours accrued between the

21st

of the month and the

20th

of the following month. The rate of pay is a flat rate. There are no enhancements for working bank holidays.

Holiday entitlement from 1.4.2021 is 5.6 weeks based on 3-day week (8.20am - 9.10pm shift)

In Homes where only one wake night staff is employed and in any case at the sole discretion of the Company, Rehabilitation Coordinators will be expected to participate in an Active On-Call System (AOC) on particular days according to the monthly rota, for which there will be separate remuneration currently 20.00 a nightstand by payment with the National Minimum Wage applying to hours during call out.

All prOKare staff are expected to attend regular appraisals and to continually improve their knowledge and skill base. Staff will receive compulsory monthly in-service training. External courses to suit individual development profiles will be offered by the organisation at the discretion of senior management.
