Job Description

We are seeking a compassionate, reliable, and skilled

Rehabilitation Support Worker

to provide person-centered care and support to individuals living with

Acquired Brain Injuries (ABI)

. This role involves delivering a combination of

personal care

, emotional support, and rehabilitation assistance to promote independence, dignity, and quality of life in a home or community setting.

Key Responsibilities:

Provide

personal care

such as bathing, dressing, grooming, toileting, and assistance with mobility. Support with transfers, including the use of hoists or other mobility aids as required. Administer medications and assist with healthcare routines as directed by a healthcare professional. Implement structured rehabilitation activities as outlined in individualized care/support plans (e.g., memory exercises, cognitive stimulation, physical exercises). Support clients in daily living tasks such as meal preparation, feeding, housekeeping, and laundry. Promote and encourage independence, choice, and participation in social and community activities. Monitor and report any changes in the individual's physical or mental health to the supervising therapist or case manager. Maintain accurate and timely records of care provided and progress toward goals. Work collaboratively with family members, therapists, and other support professionals.

Requirements:

Experience working with individuals with

Acquired Brain Injury

or other neurological conditions. Knowledge of brain injury-related challenges, including cognitive, behavioral, and physical impairments. Previous experience providing

personal care

in a healthcare or homecare setting. NVQ Level 2 or 3 in Health and Social Care (or equivalent) - desirable. First Aid, Manual Handling, and Medication Administration training - preferred. Strong communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team.
