To take responsibility for, and effectively supervise the day to day running of the home in the absence of the Home Manager/ Deputy Manager, and ensure that the care is of a high standard at all times.
JOB DESCRIPTION:
To assist the Service Users with their personal care needs and to help provide health supervision through direct nursing care when needed.
To be the named nurse for a specific number of Service Users. To take responsibility for those care plans by assessing, implementing and evaluating each service users medical, psychological and social needs on a regular basis or as needs change.
Ensure that care plans for new service users are completed within 48 hours of admission.
Identifies resident care requirements by establishing personal rapport with potential and actual residents and other persons in a position to understand care requirements.
Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
Promotes resident's independence by establishing resident care goals; teaching residents, friends, and family to understand condition, medications, and self-care skills; answering questions.
Assures quality of care by adhering to therapeutic standards; measuring health outcomes against residents care goals and standards; making or recommending necessary adjustments.
Resolves residents problems and needs by utilizing multidisciplinary team strategies.
Maintains safe and clean working environment by complying with procedures, rules, and regulations.
Protects residents and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
Documents residents care services by charting in residents care plans.
Maintains continuity among care team by documenting and communicating actions, irregularities, and continuing needs.
Maintains residents confidence and protects operations by keeping information confidential.
Ensures operation of equipment by completing preventive maintenance requirements; following manufacturer's instructions; troubleshooting malfunctions; calling for repairs; maintaining equipment inventories; evaluating new equipment and techniques.
Maintains professional and technical knowledge by attending educational workshops.
Maintains a cooperative relationship among health care teams by communicating information; responding to requests; building rapport; participating in team problem-solving methods.
Contributes to team effort by accomplishing related results as needed.
To store all Service Users care plans as Company policy and procedure and in line with Data Protection.
To lead and supervise a team of care assistants in carrying out their key worker roles.
To offer consultation and supervision to all staff on a regular basis in accordance with the Company Supervision Policy.
To ensure that all new staff are properly inducted into their roles and are mentored appropriately.
Ensure that the staff allocation sheet for the shift is completed in full and offers the correct skill mix to meet the needs of the service users.
To participate in appropriate learning and practice activities that maintains and develops your competency and performance.
Ensure coverage of the home at all times, making the best use of available staff resources.
To administer medication as prescribed by a Doctor, in line with guidelines set down by the Nursing and Midwifery Council.
Ensure stock levels are adequate and discontinued drugs are returned to the pharmacy at the earliest possible opportunity.
To complete medication administration records legibly and immediately medication has been given.
Monitor all aspects of patient care, including diet and physical activity
Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate residents care plans.
Document and communicate actions to maintain continuity among the nursing team
Comply with procedures, regulations, and rules in maintaining medical records
Maintain accurate, detailed reports and records.
Monitor, record and report symptoms and changes in residents' conditions.
Consult and coordinate with health care team members to assess, plan, implement and evaluate residents' care plans.
Order, interpret, and evaluate diagnostic tests to identify and assess resident's condition.
To be involved in the development of activities for service users to enhance their quality of life.
To oversee kitchen staff to ensure that the quality of food served meets the standard required.
To assist in the serving of meals to service users, monitoring food and fluid intake.
To oversee domestic, maintenance and laundry staff to ensure that all areas of the home are properly cleaned, maintained, adequately heated and lit.
To actively participate in Staff meetings, staff training and development activities.
To support the Home Manager in their duties under the Health & Safety at Work Act legislation and Fire Regulations.
To work within COSSH guidelines.
To read and implement the Company Policy & Procedure on Infection Control.
To have an understanding of Company Safeguarding Vulnerable Adults Policy & Procedure.
To have an awareness of the Fundamental Standards and regulations upon which the home is inspected by CQC.
To attend Mandatory training as directed by the Home Manager.
To be aware of and work in accordance with the Homes Policies & Procedures.
