Provide general medical services to patients registered with the practice. Undertakea range of consultations including face to face, telephone, online, and home visits where appropriate. Assess, diagnose, treat and manage patients medical conditions. Request, interpret and act on diagnostic tests and investigations.
Provide appropriate clinical follow-up and referrals to secondary care when required. Provide health education, preventative advice, and promote healthy lifestyles. Support the management of patients with chronic disease and long -term conditions. Work collaboratively with the practice nursing and administrative teams Contribute to QOF,IIF , PCN and enhanced services.
Participate in Practice meetings and contribute to service development. Maintain accurate up to date and legible clinical records. Review action and sign repeat prescriptions and test results. Complete medical reports, forms, and insurance documents in a timely manner.
