Job Description

Job Summary The post holder will provide a portfolio of general medical services; including managing a clinical team, managing a caseload or managing specific Groves Health wide project, ensuring the highest standards of care for all registered and temporary patients, ensuring clinical effectiveness and quality. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other healthcare working within the organisation. Ensuring that internal and external quality standards are met and maintained, including standards laid down by the Care Quality Commission and commissioners. Developing an effective team building, working relationships with clinical supervisors or mentors and other colleagues.



Taking a lead role on CQC compliant projects, business and service development and maintaining a culture of continuous quality improvement. Key Working Relationships Internal Partners, Nurses and medical lead, Caldicott Guardian Clinical Directors and Managers Team Leaders and teams/staff within Groves Health Shared Servicecs External NHS England, CCG, NHS provider/partner organisations, GP Federation and other relevant NHS bodies CQC, Royal Colleges and other regulatory or professional bodies Public Health England and other local authority bodies or functions Patient Liaison Group, third sector agencies and other key interest groups/stakeholders Key Responsibilities: Management of Patient Health & Illness Cover all the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate. To analyse and interpret history, presenting symptoms, physical findings and diagnostic information to develop appropriate differential diagnosis. To prioritise health problems and intervene appropriately, including initiation of effective emergency care.



To formulate an action plan based on scientific rationale, evidence based standards of care and practice guidelines. To use National, Community / Public Health assessment information and guidelines, when evaluating patient needs, initiating referrals, co-ordinating care and programme management. To develop and utilise a follow up system within the practice to ensure patients receive appropriate services and evaluate same. To provide counselling concerning drug regimes, side-effects and interactions and - if legally authorised - prescribe medication based on efficacy, safety and cost, from the appropriate formulary.



To order, perform and interpret common screening and diagnostic tests. To communicate the health status of the patient to the relevant health care professionals. To provide health promotion and disease prevention services to patients and their families. To maintain accurate and contemporaneous records, utilising the practice's computer system and considering the Caldicott Principles in relation to all data handling.



Act upon most alerts (e.g. QOF) and reminder during a routine consultation. Undertake surgeries during extended hours in accordance with the access needs of the practice population and the contractual obligation of the practice. Provide cross over, where necessary, to other Groves Health practices.



Respond to medical problems presented by patients, including history taking, examination, investigation, diagnosis, treatment and referral where appropriate. Provide preventative health care and advice. Provide medical support as required to patients referred by nursing staff. Provide free certification in line with Schedule 9 of NHS GMS Regulations (1992).



Adhere to the practice prescribing formulary, local and national guidelines. Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation. Records clear and contemporaneous IT based consultation notes to agreed standards. Collect data for audit purposes.



Compile and issue computer generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible. In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working in primary care.
